✔ A Letter of Medical Necessity (LMN) that includes:
	✔ Patient name ✔ Diagnosis or medical condition ✔ How the hyperbaric chamber will help ✔ Duration of treatment ✔ Doctor’s signature and contact info

2. Purchase Your Hyperbaric Chamber

3. Submit Documentation to Your FSA Provider

Send your FSA administrator:

Your invoice/receipt
Your Letter of Medical Necessity or prescription

Most FSA providers accept documents via:

How Will You be Reimbursed?

Once approved, you’ll receive reimbursement directly from your FSA provider for eligible costs. The reimbursement time frame depends on your provider’s policies.
